The Hospice Team...

Medical

The Hospice Medical Director certifies patient admissions and consults regularly with the patient care team. The patient's attending physician must approve entry into the Hospice program and will remain involved in the patient's care.

Nursing

Visits by registered nurses and home health aides vary according to the very personal needs of each patient and family.

Social Work

Social workers focus on the day-to-day needs of the patients and families. These needs may include counseling, helping with advance directives and finding resources for financial concerns and sitters.

Spiritual Care

Support and guidance are available, if desired, through clergy designated by patients and families or through chaplains trained in pastoral care.

Volunteer Services

Trained Hospice volunteers help with a variety of needs. Volunteers may provide respite for family members, emotional support or companionship.

Bereavement Care

Support Groups are an effective way to help bereaved people heal from their losses. They provide emotional, physical and spiritual support in a safe, caring environment. As participants give and receive support, they feel less helpless and are able to find continued meaning in life. One of our support group participants confirmed this by saying, "Being able to share my grief with others has given me hope that my life will get better."

Our Healing Grief series is a 4-week series that assists bereaved individuals in understanding the grief process as they work through their loss. Participants receive reassurance that their feelings of grief are normal and are able to connect with other people who are experiencing similar losses.
